WHAT:
All students must be vaccinated, or receive an approved exemption, for measles, mumps and (if under 22 years old) meningitis ACWY.
- Measles and mumps immunization: All students must submit proof of two measles and mumps vaccinations, two MMR vaccinations or lab evidence of positive titer.
- Meningitis ACWY immunization: All students under age 22 must submit proof of a conjugate meningitis ACWY vaccination given at age 16 or older.
The COVID-19 vaccine is no longer required for most students. Staying up to date on COVID-19 vaccination remains strongly encouraged and is an important public health measure to reduce the risk of disease transmission and severe illness. Please follow the University’s COVID-19 Public Health Flowchart if you develop symptoms, test positive or are exposed to COVID-19. See the Immunization Requirements website for more details.
Health sciences students participating in the Health Sciences Immunization Program are not affected by this policy change and will continue to follow all vaccination requirements communicated by HSIP program staff.
WHY:
The immunization requirements protect you, your peers and vulnerable members of the UW community.
WHEN:
Submit your records as soon as possible to avoid the risk of a registration delay. For a list of deadlines, visit the Immunization Requirements website.
HOW:
Submit your documentation to the secure immunization portal, accessed from the Immunization Requirements website.
Review the requirements carefully and upload either your immunization records or the Immunity Verification Form on the immunization portal. If you submit the form, it must be signed by a medical professional.
