Seasonal flu shot clinics for UW faculty and staff. Free if covered by insurance, otherwise $30. The next clinics are:

11 a.m. to 1:30 p.m. Oct. 12 at UW Tacoma,
11:30 a.m. to 1:30 p.m. Oct 13 at UW Bothell, and
10 a.m. to 3 p.m. Oct. 14 at UW Health Sciences.

Bring a filled-out claim form along with you. You can download the form from this website.
