Complete the Extension form and attach to a SAGE Modification Request (OSP/GCA).

Follow Extension instructions under a Schedule Change on the Award Changes webpage.

Make sure to include other relevant attachments, Grant ID#s, and any comments in the Modification request that will help OSP & GCA review and process your request.
