Suicide Risk Mitigation Plan Supplement
This supplement should be used with the GUIDANCE Participants at Risk for Suicide to describe a plan for monitoring, assessing and responding to potential risks for research in which subjects are likely to disclose that they are experiencing suicidal thoughts or behavior. If there is a Suicide Risk Mitigation Plan already described elsewhere, such as a DSMP, that may be submitted instead of this supplement, so long as it provides all the relevant information requested in the supplement.
