University of Missouri Grade replacement policy      
University of Minnesota - Twin Cities Each college has its own policy.  Some use GPA and some use coefficient of completion.Coefficient of completion can be a more lenient way of judging satisfactory academic progress than using just GPA. For example, a student who takes four 4-credit courses and completes three with a grade of C and one with a grade of F will have a coefficient of completion of 75 percent, which is satisfactory in CLA, but a GPA of less than 2.0, which is not satisfactory in most colleges.
Students suspended must wait out one year. Upon return to the unit after petitioning to reenter, all units shall use a hold and contract for the purpose of monitoring the student's performance. If the student does not successfully complete the contract, he/she shall be suspended again, but then shall be required to reapply for admission, rather than petition to reenter.
Undergraduate students must register for a minimum of 13 semester credits each semester.  Petition possible.depending on circumstances. Same tuition for 13 or more credits.  Everyone pays for 13 credits unless an exemption for less than 13 credits is granted.   13 credit rule designed to encourage students to graduate in 4-5 years.
University of Michigan - Ann Arbor Action Pending: assigned when a student's academic record for a term is incomplete and the student is in danger of completing the term with less than a 2.0 grade point average

University of California - Berkeley Students are placed on academic probation if their cumulative UC GPA falls below 2.0. Need to bring  cumulative GPA up to a 2.0 the following semester  else subject to dismissal. Students are also placed on academic probation if their Berkeley term GPA falls below 1.5 for any semester. Must bring term GPA up to a 2.0 in the following semester, or subject to dismissal.

13 units per semester is minimum course load allowed without special approval. Must declare a major by the beginning of junior year or registration may be blocked. Transfers must declare a major by the beginning of second semester.

Cincinnati There are no specific university policies for these categories.  There is no Satisfactory Progress Policy at this time since the University is on a campaign to increase enrollment.

Florida Must reapply for admission if dismissed due to poor performance.  Must maintain a 2.0.  Students dismissed with a grade point deficit of more than 15 points (grade points below a 2.0). Must maintain the minimum GPA and "reasonable conformance to a program of study    
Iowa Students with 0-29semester hours must earn a UI and a cumulative GPA of 1.85.  Other students must earn a 2.0 4 Year Graduation Plan - sign an agreement and University guarantees completion in 4 years    
Kentucky Students in the College of Arts and Sciences are placed on probation of the cumulative GPA falls below 2.0.  If students have 2 consecutive terms with a GPA below 2.0, regardless of cumulative, student is on probation.  Students on probation are required to meet with an adviser   Students must declare a major by the time they have earned 60 credits.  A registration hold will be placed until a major is declared.  Transfer student with  60 total credits must declare a major prior to the next registration period.  
University of North Carolina Minimum cumulative GPA of 1.00 for any semester 1.50 GPA  & 24 credit hours after 2 semesters
1.75 GPA  & 51 credit hours after 4  semesters
1.90  GPA & 78 credit hours after 6  semesters
2.00  GPA & 105 credit hours after 8 semesters
Permission from the appropriate officials must be secured before a student can attend 10 semesters.  For General College and Arts & Sciences, students are required to check academic eligibility on the web (or by phone if they have no access to the web).  (This is a new policy.)
Entering students are admitted to the General College.  Junior and Senior students are admitted to the College of Arts & Sciences, unless they have been accepted by a professional school.  Students typically declare a major the spring of their sophomore year.  Students may only pursue one bachelor degree at a time. 120 semester hours with a 2.00 GPA.  The 2.00 GPA must be achieved in no more than 45 credits in excess of graduation requirements for the degree program. 24 of last 30 must be taken in residence (including distance and UNC study abroad programs).  For most degree programs, students must pay a 25% surcharge on credit hours taken past 140 semester hours. Seniors are asked to check with a Senior Team advisor to check on graduation requirements.
University of Pittsburgh Academic standing is determined by the student’s school or college.
For Arts & Sciences, a minimum cumulative GPA of 2.00 for each semester enrolled.  Students with less than a 2.00 are put on academic probation.  Two straight terms of less than 2.00 GPA results in suspension or dismissal
For full time students,
1. 50 GPA after 23.0 credits 
2.00 GPA after 53.0 credits
2.00 GP after  83.0 credits
2.00 GPA after 113.0 credits
Students who have not completed a degree after 10 full-time semesters are considered to have not made satisfactory progress.
Entering students not admitted directly into a professional school, such as Engineering or Nursing, are admitted to CAS when they are admitted to the university.  Students without a major must make appointments to see their advisor twice a semester.  They typically declare a major spring of their sophomore year. Graduation requirements differ with each school, but all schools require 120 credits with a cumulative 2.0 GPA.
University of Wisconsin No general policy.  In the College of Letters and Science, students who achieve less than a 2.0 GPA are put on probation the first time. If they achieve less than a 2.0 on probation, they receive strict probation or are dropped for a year. Students must achieve the minimum standards as set by each school or college.  For College of Letters and Science, this is a 2.0 GPA for each term There is no general policy on declaring majors.  In the College of Letters and Sciences, juniors and seniors must declare a major.  Students may change their major as much as they want, but must fill out a major/certificate form each time. Students must complete at least 120 credits with a cumulative GPA of 2.00.  Four Year Graduation Agreement:  a contract between the student and the university that provides a limited guarantee that the student will be able to graduate in four years.  Wisconsin’s accountability report includes information on “Average Attempted Credits to Degree.”
University of Utah Students who have less than a 2.0 GPA for three terms while their cumulative GPA has continued to be under 2.0 may be suspended for two terms. Students must maintain a minimum cumulative GPA of 2.0 Entering students are admitted to the University College.  They are expected to declare a major after their sophomore year (when they have earned 60 semester credits). Students must complete a minimum of 122 semester credits with a 2.0 GPA, 60 credits of which must be upper division Students are allowed 183 attempted credit hours to complete a degree. (Pharmacy students are allowed 233.)
University of Virginia To maintain Good Standing, students must earn at least 12 hours of course with at least a 1.800 semester GPA, and no more than one grade below C-.  Students who don’t go on are placed on Academic Warning.  Students who are on Academic Warning for two consecutive semesters are subject to suspension. To enroll in a fifth semester, students must have earned at least 54 semester credits.  For a seventh semester, students must have earned at least 84 credits.  Students must make up credits in the School of Continuing and Professional Studies or in a Summer Session. Students must be in a major or have completed a Deferral of Major Form by the end of their fourth semester.  The Deferral of Major Form is good only until the end of the fifth semester. Students entering from secondary school are expected to complete the requirements of a degree in 8 semesters.  Unless they receive written approval from the Dean, students must complete any remaining degree requirement in the School for Continuing and Professional Studies, the Summer Session, or at another institution.
UC - Davis A student will be placed on probation for qualitative reasons if, at the end of any quarter, the student’s grade point average (GPA):
* is less than 2.0, but not less than 1.5, for the quarter, or
* is less than 2.0 for all courses taken within the University of California
A student will be subject to disqualification for qualitative reasons if, at the end of any quarter,
* the student’s grade point average is less than 1.5 for the quarter,or
* the student’s grade point average is less than 1.5 for all courses taken within the University of California.or
* the student has attempted more than 16 units graded I (Incomplete),or
* the student has spent two consecutive quarters on academic probation.
A student will be subject to disqualification for quantitative reasons if the total number of units passed at UC Davis is less than 39, calculated at the end of every quarter for the preceding three quarters of enrollment.
The dean of the student’s college may grant a student a minimum progress variance of one or more quarters for an acceptable reason.
Minimum progress requirements do not apply to students who have part-time status or to students who have their dean’s approval to carry less than the minimum progress load because of medical disability, employment, a serious personal problem, a recent death in the immediate family, or a serious accident involving the student.
College of Agricultural and Environmental Sciences. Students must declare a major by the time they have completed 120 units. Failure to declare a major at this point may result in a hold on further registration.
If you have completed 120 units, you must prepare a study plan with your adviser and/or advising associate at the same time. You are accepted into a major only after your major department and the dean’s office have approved the Change of Major petition.
College of Engineering. Students must declare a major when they apply to the College of Engineering. Their freedom to change majors thereafter may be limited.
College of Letters and Science. Students must declare a major by the time they have completed 90 units. If you have not declared a major by this point, a hold may be placed on your registration.
As a part of the declaration procedure, you must, in consultation with a faculty adviser, prepare a projected plan of study.
You are normally expected to fulfill all degree requirements within the 180- to 225-unit range. Once 225 units have been completed (excluding units awarded for College Board Advanced Placement Examinations), you may register only with the permission of the dean. Such permission is rarely granted, and then typically only to allow completion of minimum degree requirements. You will be expected to adhere to a program of courses agreed upon and to meet other conditions that may have been set. Approval must be obtained before you will be permitted to register for courses for the quarter following completion of 225 or more units.
If you are in good standing, you will be able to complete 12 quarters or the equivalent (e.g., four years) of college work even if you have earned more than 225 units before you finish your fourth year. You must petition for continuation, however, and file the quarter-by-quarter course program you have planned.
UC - Irvine An undergraduate student normally is subject to academic probation if at the end of any quarter the grade point average for that quarter, or the cumulative grade point average, is less than 2.0.
A student whose grade point average falls below a 1.5 for any quarter, or who after two consecutive quarters on probation has not achieved a cumulative grade point average of 2.0 or a satisfactory rate of progress, is subject to disqualification.
Please see attachement. All students are required to declare a major by the time they reach junior status (90 units excluding college work completed prior to high school graduation) or they will become subject to disqualification from further registration in the University. All undergraduate students are expected to graduate when they have completed the baccalaureate requirements of their declared major or majors.
UC - San Diego An undergraduate student is subject to academic probation if at the end of any term his or her GPA for that term or his or her cumulative GPA is less than 2.0.
An undergraduate student is subject to academic disqualification from further registration if at the end of any term his or her GPA for that term is less than 1.5 or if he or she has completed two successive terms on academic probation without achieving a cumulative GPA of 2.0. Continued registration of an undergraduate who is subject to disqualification is at the discretion of the faculty of the student’s college or its authorized agent.
A full-time undergraduate student is subject to disqualification from further registration if he or she does not complete thirty-six units in any three consecutive quarters of enrollment. Continued registration of an undergraduate who is subject to disqualification due to lack of minimum progress is at the discretion of the faculty of the student’s college or its authorized agent (generally the provost/Office of the Provost). Eligible students may file for an exemption from the minimum progress requirement by completing the Part-time Study application and receiving college approval prior to the end of the second week of the quarter. Upon enrollment, student must choose one of six colleges with which to affiliate, even if they haven’t yet chosen a major.  They must identify a second or third choice in the event the first choice college closes early.  Enrollment may be limited in certain majors, therefore a second choice major must be identified by the student.  Premajor requirements are identified in selected fields.  Students must complete them prior to admission to these majors.  Regardless of the college they affiliate with, students may enter any major. An undergraduate may register for no more than 200 course units.  An exception is permitted for candidates for B.S. degrees in engineering, for whom the limts are 240 units in Revelle and Roosevelt Colleges and 230 units in all other colleges.  Other exceptions will be granted only for compelling academic reasons and only with the approval of the college provost and the concurrence of the Committee on Educational Policy.
Transfer units applicable toward general education requirements or major requirements are included in the maximum unit calculation; all other transfer units are excluded. Advanced Placement and international baccalaureate units are excluded.
UCLA Academic Probation
You will be placed on probation if your term/overall GPA falls below 2.0 but is above or equal to 1.5.
If placed on probation, you will have two terms to get your overall GPA back to 2.0
If your term GPA falls below 2.0 while on probation, you will become Subject to Dismissal
If you do not bring your overall gpa to 2.0 or above within two terms, you will become Subject to Dismissal

Students who are either on Academic Probation or who are Subject To Dismissal may not take classes on a Pass/No Pass grading basis.
Please see attachment. Be aware that certain major departments maintain unit limits by which the major must be declared; i.e., you must declare the major before you have completed a certain number of units. Please consult with the individual departmental counselors to determine the prerequisite coursework that you need to complete and the specific grade point average requirements.  
University of Hawaii - Manoa Students may be placed on academic probation at the end of any semester in which their cumulative GPA falls below 2.0 or in which they fail to maintain the minimum academic requirements of their college, school, or program. Probationary students continue work at the University but must achieve a current GPA of at least 2.0 in each probationary semester to be allowed further registration. Failure to meet these conditions may result in suspension or dismissal.
Students may be suspended when they fail to achieve a cumulative GPA of at least 1.7 after attempting 24 credit hours or when they fail to meet the terms of probation.
Students readmitted after suspension are placed on probation and must meet the terms and conditions of probation as specified above. Failure to do so will result in dismissal.
Students who have been suspended and who subsequently fail to maintain the minimum academic requirements of the University or their college, school, or program or fail to meet the terms of probation may be dismissed.
Once dismissed, a student is not eligible
for readmission to any degree- or
credit-bearing program at the University of
Hawai‘i at Manoa for a minimum of one
academic year.
Unless an extension has been granted by the college or school’s student academic services dean, students may be put on probation if they have taken 24 credit hours beyond those required for graduation but still have not completed their specific program requirements.
  A student who by the end of any semester has earned 24 credit hours beyond those required for graduation and has fulfilled all specific program and University requirements may be graduated by action of the student’s college or school.
Cornell University Engineering Advising Office contacts students identified as having trouble.  If a student does not maintain this standing, there is a
Committee on Academic Standards, Petitions, and Credit, which can issue a
"warning," a "stern warning," require a student to take a leave of absence
or make the student withdraw from the college.
The College of Arts & Sciences at Cornell has a similar policy for
low scholarship.  A faculty Committee on Academic Records reviews all of
the students not in good standing and decides what to do with them.  "Not
in Good Standing" for the college is defined as: failing to complete 12
semester credits in a quarter, having a failing or unsatisfactory grade,
having two or more D's or having one D in a schedule of only three
courses.  The committee may use one of the following options (all of
which are noted on the students transcript): a warning (which does not
appear on the transcript), required leave of absence (usually for a year, or required withdrawal.
The Engineering college also has a first semester freshman satisfactory progress rule (12 credits a semester, C- or better in math, semester average of 2.0 or higher, and no failure or incomplete grades) and a second semester freshman/first semester sophomore requirement (14 credits a semester in engineering degree requirements, and the rest is the same as above). Cornell does have competitive admission majors.  Cornell also has open admission majors.  Declaration of the major varies depending on the college.  Cornell has a College of Arts & Sciences (where students must declare a major by the end of the second year) and four "Professional Colleges".  In the College of Engineering, students should declare by the end of the third quarter.  I think that what happens in Engineering, is that a student isadmitted into the college of engineering from almost the start, but they declare an affiliation with a certain field of study in engineering (mechanical, chemical, etc.) by the end of the third semester of study. The degree completion policy is handled by requiring students to take a certain number of credits or classes each quarter that count towards their degree.  The College of Arts & Sciences even encourages time off (15% of their students take time off) to explore.  Although Arts & Sciences says they don't look at graduation rates until five years after matriculation, they also say that most students finish in 8 semesters of work.
Michigan State University Please see attachment. Integrated studies must be done first year, pre-reqs for major must be done second year, and students enter major at beginning of Junior year.  If a student is not ready for their major at the beginning of their Junior  year, they must select another major for which they are ready.  Student is no longer able to register unless they are in the major, once they have earned 60 semester credits. (Incidentally, students are charged more at the Jr. and Sr level) Two kind of majors, Restricted enrollment, and Non-restricted enrollment.  Freshman declare major upon entering the University, but don’t actually enter that major until their Junior year.  
Ohio State University A student will be dismissed from the College if his/her grade point average is below the following levels:  After attempting 15 credit hours, 0.75 (except first qtr students); and after attempting 30 credit hours, 1.5; after attempting 45 credit hours 1.75; after attempting 75 credit hours 1.9; after attempting 80 credit hours 2.0
A student who has been dismissed from the College for failure to maintain minimum scholastic requirements must:
1. Remain out of COTC fro at least one quarter (excluding Summer Quarter, unless Summer Quarter is required in the major area of study).
2. Complete a petition  (available at the Student Records Office) for a consideration of reinstatement to the Academic Board at least four weeks prior to the beginning of the quarter in which the student wishes reinstatement. 
3. The student must also appear before the Academic Board to personally request for reinstatement.
     
Texas A & M Mandatory Aadvicing every semester for every student.

University of Arizona Below 2.0 mandatory advising, below 1.6 minimum gpa, they go on probation-all probationary freshman get 3 semesters to get above a 1.6, before getting dropped.  If dropped, the must go to winter or summer session to get gpa up to a 2.0, OR go to CC, earn 12 credits, and 2.5 and they can come back, on probation.
If out for 5 years, they come back on ‘academic renewal’, they keep anything that was a C or better and drop everything else.
  Most liberal are majors are open, student can declare upon admission. Over 60 credits-registration hold-Must choose a major at this time, no ifs ands or buts, Most majors do not have prereqs, so this is not a problem.
When a student earns 87 credits, they must finalize graduation plans, they are blocked from registration until this is done, departmental advisers see this group.  Students may stay as long as they want, there is no limit time to graduation.
Washington 4-Year Schools
Western Washington University A warning is issued to a first-quarter freshman whose grade average is below 2.00 and to any continuing student whose quarterly grade average is below 2.00 but whose cumulative grade average is 2.00 or higher. 
Any student, except a first-quarter freshman, whose cumulative grade average falls below 2.00 is placed on academic probation.
A student who begins a quarter on probation and, during that quarter, earns a grade average of 2.00 or higher without raising his or her cumulative grade average to at least 2.00 is placed on continuing probation. The student must then improve his or her cumulative grade average to at least 2.00 or attain at least a 2.30 quarterly average during the next quarter of  enrollment.
A student will be dropped from the University if he or she (a) begins a quarter on probation and earns a quarterly grade average below 2.00 or (b) begins a quarter on continuing probation and fails to raise his or her cumulative grade average to at least 2.00 or, alternatively, fails to attain at least a 2.30 quarterly average.
  Students must declare their major by the time they have completed 120 credits or a hold is put on their transcript. Students who reach 210 credits without graduating or submitting a degree application or plan of study will not be permitted to register.
Eastern Washington University Students whose cumulative GPA is less than 2.0 at the end of a quarter are put on probation.  New students at Eastern are subject to dismissal after two quarters of probation.  Continuing students are subject to dismissal after one quarter of probation. No general policy, except for purposes of financial aid. Students must declare their major by the time they complete 95 credits.  Students with less than 90 credits and no major are assigned advisors in General Academic Advising. 180 credits with a cumulative GPA of 2.00 in all coursework.  60 credits must be upper division, and 45 credits must be taken in residence at Eastern.  A minor is required if the major requires less than 60 credits.   Eastern has four year guarantee programs, which require declaring a major before the beginning fall quarter as a freshman.
Washington State University A student who falls below a 2.00 cumulative GPA or who falls below a 2.00 semester GPA for two consecutive semesters is considered academically deficient.  Students whose cumulative GPA is less than 2.00 for two semesters are normally dropped No general policy, except for purposes of financial aid. Student may “certify” a major after completing 24 semester hour credits, and must by the time they complete 60 semester hour credits.  After completing 60 semester hour credits, they may certify a second major or minor.  Once certified, a student cannot be decertified for a major unless he or she becomes academically deficient (defined aboved).  Transfer students who have more than 60 hours credit must spend one semester being counseled by the Student Advising and Learning Center. 120 semester credit hours with a cumulative GPA of 2.00, and at least 40 semester credit hours of upper division coursework.
Central Washington University Academic Warning: A student who has been in good standing will be placed on academic warning when the GPA for the previous quarter is below 2.0.
Academic Probation: A student who has been on academic warning will be placed on academic probation if either the quarterly or cumulative GPA is below 2.0.
Academic Suspension: A student who has been on academic probation will be placed on academic suspension if the GPA for the previous quarter is below 2.0. If the GPA for the previous quarter is 2.0 or above, but the cumulative GPA remains below 2.0, the student will remain on academic probation. Immediately after grade reports are prepared, the Vice President for Student Affairs reviews the academic files of all suspended students and makes one of three decisions:
* The student may be allowed to register for one more quarter with an academic standing of probation.
* The student may be allowed to submit a petition presenting evidence of circumstances beyond the student's control which adversely affected the student's performance during the preceding quarter(s).
* The student may be denied enrollment for
one year. Readmission, however, is not
guaranteed
No general policy except for purposes of financial aid Students who have earned 100 or more credits and who have not applied and been admitted to a major or pre-major will not be permitted to register for classes until they submit a completed report of pre-major advising to the Office of the Registrar indicating that they have discussed a major with a faculty advisor. Otherwise, students who have not been admitted to a major or pre-major prior to 100 credits will be given last priority during early registration. A minimum of 180 quarter credits with a cumulative GPA of 2.00.  At least 60 credits must be upper division coursework.  A student must have a 2.25 GPA in the major field, including in courses accepted for transfer.
The Evergreen State College Academic Warning: A student who earns fewer than three-fourths the number of registered credits in two successive quarters will receive an academic warning. A student will be removed from academic warning status upon receiving at least three-fourths of the credit for which she or he is registered in two successive quarters.
Required Leave of Absence: A student who has received an academic warning and while in warning status receives either an incomplete or fewer than three-fourths of the credit for which she or he is registered, will be required to take a leave of absence, normally for one full year. A waiver of the required leave of absence can be granted only by the academic dean responsible for academic standing upon the student's presentation of evidence of extenuating circumstances.
  All students plan their own majors at Evergreen A minimum of 180 quarter credit hours.  For the Bachelor of Science, 72 quarter credit hours must be credit equivalencies of  science, computer science and/or mathematics.  48 of those 72 hours must be upper division coursework.
