To protect your confidentiality, the survey does not autosave. Please make time to finish in one sitting; if you need to step away and your device goes to sleep, you may have to start over. The survey takes about 20–30 minutes to complete.
Please note: To help us understand your range of experiences at the UW, this survey includes potentially sensitive questions about sex-based and gender-based harassment, violence and discrimination. You may skip any questions you don’t want to answer — the only two required questions are about your primary role (e.g., student, employee) and your campus or UW Medicine location. Remember to click “Submit” at the end of the survey so that your answers are recorded.
Look for initial results in fall 2025, with leadership response and action items identified in winter 2026.
